On October 18, 2024, Madison Bruce Smith, just four months old, was found unresponsive by his father, Matt Smith, at their home in Trafford, Greater Manchester. Despite efforts to resuscitate him, he was pronounced dead at Wythenshawe Hospital.
The circumstances surrounding Madison’s death have raised serious concerns regarding the regulation of maternity services. An unregulated maternity nurse had advised Madison’s parents to place him in a prone sleeping position, contrary to NHS guidelines. This tragic decision ultimately led to devastating consequences.
“Losing Madison has been utterly excruciating. The pain is indescribable and often too much to bear. It has totally shattered our entire family,” expressed Alex Smith, Madison’s father, as he mourned the loss of his son.
Coroner Alison Mutch highlighted the alarming lack of regulation in the maternity services sector, noting that the head of Ruthie Maternity Services had no medical qualifications, possessing only a first aid certificate. This revelation has sparked outrage and calls for reform.
In response to Madison’s death, the coroner issued a prevention of future deaths report to the Secretary of State for Health, emphasizing the urgent need for better oversight in childcare. The National Nanny Association has also joined the call for mandatory DBS checks for childcare providers to ensure the safety of infants.
