Noelia Castillo Ramos, a 25-year-old woman from Barcelona, Spain, tragically passed away on March 26, 2026, after choosing euthanasia. Her life was marked by profound trauma, including her parents’ divorce and multiple incidents of sexual abuse, which contributed to her mental health struggles. Diagnosed with obsessive-compulsive disorder (OCD) and borderline personality disorder (BPD), Noelia faced overwhelming challenges that led to a suicide attempt in October 2022, leaving her paraplegic.

In July 2024, Noelia’s request for euthanasia was initially approved, but it faced significant legal hurdles, particularly from her father, Geronimo Castillo, who argued that her mental illness impaired her ability to make such a critical decision. This case became the first in Spain to go to trial regarding euthanasia, a practice that was legalized in the country in 2021 for individuals suffering from incurable or severely debilitating conditions.

Throughout her struggles, Noelia expressed her desire to end her suffering. “I want to go now in peace and stop suffering, period,” she stated during a television interview. Her poignant words resonated with many who understood the depths of her pain. Despite her family’s opposition, she remained resolute, saying, “He hasn’t respected my decision and he never will.”

On the day of her euthanasia, supporters gathered outside the hospital, hoping to change her mind. However, Noelia was determined to proceed, stating, “I just want to go in peace and stop suffering.” The procedure involved three injections, with the final one resulting in cardiac arrest.

Her case has sparked intense debate about the ethical implications of euthanasia, particularly concerning mental health. Observers note that while Spain has made strides in legalizing euthanasia, the complexities surrounding mental illness and the decision-making capacity of individuals like Noelia remain contentious issues.
